You May Be A Candidate If You:
- Struggle to eat, smile or live confidently

- Currently wear dentures or have been told you may need them
- Believe full-arch dental implants could improve quality of life

Who can apply for Smile It Forward?
Smile It Forward is intended for adults who are living with significant tooth loss or failing teeth and may benefit from full-arch dental implant treatment. You don’t need to know whether you’re a candidate before applying. If you think this opportunity could be right for you, we encourage you to apply.
Do I need to have an extraordinary story to apply?
No. Smile It Forward isn’t about finding the person with the most difficult story. We want to hear from people whose dental health has affected their lives and who may benefit from a new smile.
What does the selected recipient receive?
The selected Smile It Forward recipient will receive treatment for one full arch, upper or lower, at no cost. Treatment includes:
- Consultation
- Full-arch dental implant treatment
- Extractions, if needed
- IV sedation
- Temporary prosthesis
- Final prosthesis
The recipient will be responsible for ongoing care and maintenance after treatment is complete, including Transcend’s $125 annual maintenance visit.
The specific treatment provided will be determined by our doctors based on the recipient’s individual clinical needs.
How will the recipient be selected?
Applications will be reviewed by the Transcend team. Selected applicants will be invited for an in-person consultation and clinical evaluation. The final recipient will be selected based on program criteria and clinical candidacy.
What if I don't know whether I'm a candidate for dental implants?
That’s okay. You don’t need to know before applying. Applicants who advance in the process will be evaluated by our doctors to determine whether full-arch dental implant treatment is clinically appropriate.
Does it cost anything to apply?
No. There is no cost to submit an application for Smile It Forward.
Can I nominate someone else?
Yes. If someone you care about could benefit from Smile It Forward, we encourage you to share the opportunity with them. Because the application includes personal and dental information, the individual receiving treatment will need to complete the application themselves.
Will I need to be photographed or filmed?
Because Smile It Forward is about sharing the impact of a new smile, the selected recipient will need to be comfortable allowing Transcend to document portions of their experience through photography, video and/or their personal testimonial.
